What is a Medical Record?
A medical record is a permanent medical document(s) of every patient that contains medical information and physician documentation of care. The medical record contains a collection of observations and findings recorded by the patient's treating physician and other members of the medical health clinic. The entries and reports within the medical records can also contain medical reports from other medical facilities or laboratories. Medical Records are utilized for patient care, to verify insurance claims for medical care rendered and as a legal/business a document outlining the course of a patient's medical care by the treating physician.
How do I request my Medical Records?
If you need any medical record you must submit a written request to our Medical Records department. Medical Record fees may be applied for certain medical records.
